Our courses are currently available free of charge to NHS staff in England only, with a focus on those with patient safety and investigation roles.

Our flagship programme, the CPD accredited ‘A systems approach to investigating and learning from patient safety incidents’, is on-demand learning and we can accommodate large numbers of leaners in each cohort.

The other courses are delivered online to small groups. With a limited resource, access to these must be strictly controlled to ensure sufficient availability.

Aims

The aims of our education programme are to:

  • Support a professional approach to healthcare safety investigations.
  • Enable learning from patient safety incidents.

Our education programme is agile and responds to the needs of staff who work in health and social care. We produce learning that is innovative and practical, to share our approaches to patient safety investigation and motivate staff to continue their learning.

Course housekeeping

  • Please add the course to your calendar when you register.
  • Familiarise yourself with your Canvas course page. There is lots of information about course content and delivery, including surveys that you need to complete for certification.
  • Emails about courses that you are registered for are sent from notifications@instructure.com. Please add this email to your ‘allowed’ list in Outlook or ask your IT department to help you.
  • Conference links are sent on the morning of the course date. Please check your junk email for the link if you have not received it. Alternatively this can be found on your course homepage.
  • If you cannot find your course emails, you can access your course through the Canvas website.

Patient Safety Incident Response Framework training courses

To support the NHS to further improve patient safety, NHS England has launched the Patient Safety Incident Response Framework (PSIRF). It outlines how NHS trusts should respond to patient safety incidents and how and when a patient safety investigation should be conducted.

We’ve developed a range of courses to support NHS trusts to implement and use PSIRF.

PSIRF courses:

  • Involving those affected by patient safety incidents in the learning process
  • Patient Safety Incident Response Framework Oversight

A systems approach to investigating and learning from patient safety incidents

This course is aimed at those who lead investigations and other learning responses and those in Patient Safety Incident Response Framework (PSIRF) oversight roles. The course meets the minimum training requirement of this specific element of the new PSIRF.

In this course you will learn about:

  • Complex systems, systems thinking and Human Factors.
  • Investigation practices such as interviewing, capturing work-as-done, using a systems framework (SEIPS), synthesising data and writing reports.
  • Developing effective safety actions and recommendations.
  • Engaging and involving those affected by patient safety incidents.

This is an on-demand learning course with bitesize modules for you to learn at your own pace.

As this course is CPD certified, you can record 20 CPD points upon completion.

Involving those affected by patient safety incidents in the learning process

This one-day live course is aimed at those who have a lead role in engaging and involving those affected by patient safety incidents. Those affected includes patients, family members or healthcare staff.

The day is split into four modules:

  • Creating the right foundations.
  • Before and initial contact.
  • Continued and closing contact.
  • Additional considerations.

The course meets the minimum training requirement of this specific element of the Patient Safety Incident Response Framework (PSIRF). It also aligns to the accompanying PSIRF guidance ‘Involving those affected by patient safety incidents in the learning process’. To reach the minimum training requirement you are required to attend the whole day.

As this course is CPD certified, you can record 7 CPD points upon completion.

Patient Safety Incident Response Framework oversight

This live course is designed to introduce the principles of patient safety incident response oversight. It covers the mindset and the culture underpinning effective oversight, as well as the theory and practice of measurement and monitoring of patient safety.

This is a seven hour course, delivered in two sessions of 3.5 hours each, with a gap of about a week between the sessions.

As this course is CPD certified, you can record 7 CPD points upon completion.

Investigative interviewing

This live 3.5-hour course offers an overview of the principles that underpin a professional safety investigation interview with either a member of staff, a patient or a family.

The course aligns to the PSIRF guidance on a systems approach to interviews.

As this course is CPD certified, you can record 3 CPD points upon completion.

Demystifying thematic analysis

This live 3-hour course introduces the concept and approach to thematic analysis.

We recognise that this form of analysis may be new to some staff looking to integrate the learning required when implementing the Patient Safety Incident Response Framework (PSIRF) into your organisation. However, we hope that demystifying this form of analysis will allow you to reflect on how you may already use this approach to some extent, but also how you could expand or introduce this approach to enhance organisational learning.

As this course is CPD certified, you can record 3 CPD points upon completion.

After Action Review

This live 3.5-hour course is aimed at those who plan to use, or may already be using, After Action Review (AAR) as one of their learning responses to patient safety events. It is also be useful for those in Patient Safety Incident Response Framework (PSIRF) oversight roles.
