The Healthcare Safety Investigation Branch (HSIB) were made aware of a patient who had a ruptured fallopian tube following a delay in diagnosis of an ectopic pregnancy. 

The initial investigation focused on the patient’s care pathway and safety issues were identified will form the basis of the full investigation. This includes: 

  • Referral from the emergency department into early pregnancy services
  • Provision of early pregnancy assessment services that allow for the timely diagnosis and optimum management of ectopic pregnancies
