Notification of Investigation: Provision of mental health services in emergency departments

The Healthcare Safety Investigation Branch (HSIB) was made aware of a woman who, having presented to her general practitioner, ambulance service and the emergency department of her local hospital, subsequently took her own life.

After a preliminary investigation with the full cooperation of local care providers, HSIB has decided to launch a full investigation.  The focus of the HSIB investigation will be on the provision of care to patients who present at emergency departments with mental health problems; specifically, how psychological needs can be recognised and assessed alongside physical symptoms and how timely access to psychiatric liaison services can improve the management of care and patient safety.
