Notification of Investigation: Transition from Child and Adolescent Mental Health Services to Adult Mental Health Services

The Healthcare Safety Investigation Branch (HSIB) was made aware of an 18-year-old man who, shortly after transferring from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS), took his own life.

HSIB are now launching a full investigation and the focus will be on the transition from CAMHS to AMHS taking into consideration the age, organisation and pace of transition.
