As the new year approaches it’s an appropriate time to reflect on HSIB’s progress and achievements during 2018/19, with the publication of our Annual Review.

It was a period of rapid growth, learning and development. Significantly, from 1 April 2018 we became responsible for conducting maternity investigations alongside our programme of national investigations. Our approach has emphasised the need to work closely with mothers, families, healthcare staff and organisations during our investigations.

This was recognised nationally when our family engagement model was a finalist in the ‘best emerging solution for patient safety’ category at the HSJ Patient Safety Awards 2019.

Keith Conradi, Chief Investigator, said: “The support and engagement of patients and families, providers, NHS staff and other healthcare bodies we’ve worked with has been critical for our early achievements, in our national and maternity investigation programmes, and in the success of our family engagement model.

“There has been great progress in our second year and this is testament to the hard work and commitment of our staff and supporters.”

The Annual Review 2018/19 also shares the reflections of our advisory panel, along with details of our two investigation programmes and how we’ve met our strategic objectives.
