We’ve appointed a new director of investigations to lead the continued delivery of our national investigations programme.

Dr Stephen Drage joins us from Brighton and Sussex University Hospitals NHS Trust, where he held the position of deputy medical director for six years. In this role his portfolio included oversight and development of the serious incident investigation process and implementing the Trust’s response to the National Guidance on Learning from Deaths.

Stephen said: “I’m excited to be working for HSIB, which offers a unique approach to improving healthcare in England, and influencing safety investigation across the world. My experience of working with patients and families who have been hurt by inadequacies in our health system makes me passionate about improving care by the application of knowledge gained from high quality incident investigation.”

Alongside his role at HSIB, Stephen is a practising intensive care consultant and anaesthetist with specific interests in improving care of deteriorating patients, sepsis management and acute respiratory failure.
