HSIB has launched an investigation into the delayed recognition of retained vaginal swabs and tampons following childbirth. 

The incident involves a woman who was experiencing severe pain following the birth of her child. When examined five days post-partum (after childbirth), a surgical tampon was discovered.

The investigation will focus on the on use and detectability of swabs and tampons and will explore the environmental and team factors that surrounded the incident.
