DHSC publishes HSIB East Kent Hospitals maternity summary report

In January 2020 we were asked by the Department of Health and Social Care (DHSC) to provide a summary report of all the investigations we had undertaken, up to that point, with East Kent Hospitals University NHS Foundation Trust (EKHUFT). This was due to the rising concerns around the care provided by EKHUFT’s maternity services at William Harvey Hospital in Ashford and the Queen Elizabeth the Queen Mother Hospital in Margate.

New report highlights the ‘devastating impact’ of missed ectopic pregnancy

Report highlights ‘toxic’ risk of beta blocker to prevent overdose deaths

The toxicity of a commonly prescribed beta blocker needs better recognition across the NHS to prevent deaths from overdose, our new report warns. The report focuses on propranolol, a drug that is highly toxic when taken in large quantities. Patients deteriorate quickly, making it difficult to treat.
Earlier recognition of aortic dissection needed to prevent deaths

Up to half of all patients who suffer an acute aortic dissection may die before reaching crucial specialist care, according to our new report. The report highlights the difficulty which can face hospital staff in recognising acute aortic dissection. As a result, two safety recommendations have been made.

New national investigation looks at implementation of safety improvements for the placement of NG tubes
