Healthcare Safety Investigation Branch published its first national investigation report yesterday.
This follows the release, of the Gosport Report identifying a "catalogue of failures", by numerous organisations and individuals, in the deaths of 456 people in the hospital’s care.
I commend the ‘family first’ approach taken by Bishop Jones and the Panel.
The HSIB teams engage with patients and families throughout their investigations, determined by a rigorously developed protocol. This ensures the patient voice is heard throughout the investigation and in all our reports.
At a time when confidence in the health system investigation approach may be shaken, I encourage people to read the HSIB report available here. Our commitment to, independent investigations that do not apportion blame or liability is one I am proud of and I look forward to HSIB carrying out up to 30 national investigations each year and having a positive impact on patient safety.
