Our Advisory Panel

The HSIB Advisory Panel is a group of independent experts in the fields of healthcare, patient safety, safety investigations and patient and public advocacy.

What does the Advisory Panel do?

The Advisory Panel does not have a formal governance role. However its expertise provides important guidance for the HSIB executive team on matters relating to investigations and the organisation’s strategic direction.

The panel’s main function is to provide external scrutiny and oversight of the relationship between HSIB and its sponsor agencies the Department of Health and Social Care and NHS England and Improvement, to ensure that HSIB remains functionally independent. It will also support HSIB in establishing the role and remit of the Citizen’s Partnership.

Our Advisory Panel members were appointed following an open process.

Role of the panel

Independent scrutiny

The Advisory Panel provides external challenge, critique and comment on the status, operation and outputs of HSIB as an independent body.

Stakeholder input

The Advisory Panel provides a forum to gather and consider the views, questions and interests of key HSIB stakeholders.

Expert advice

The Advisory Panel provides expert knowledge and input to the Chief Investigator and HSIB on key issues as required from professional, public and patient perspectives.

Network scanning

The Advisory Panel provides input and information to the Chief Investigator and HSIB from the collective stakeholder networks represented by panel members.

Investigation prioritisation

The Advisory Panel provides input, review and assurance on the short-term and long-term prioritisation process of selecting investigations.

Learning and improvement

The Advisory Panel provides external review and evaluation on the learning impacts and improvement outcomes of HSIB reports and safety recommendations.

Professor Murray Anderson-Wallace

Professor Murray Anderson-Wallace has a clinical background in mental health services and psychological therapy, but has spent most of his career specialising in the social psychology of organisation and applied communications research.

He is visiting professor at the Health Systems Innovation Unit at London South Bank University, where he co-leads the Darzi Clinical Fellowship Programme. He also teaches on the MSc in Quality and Safety Improvement at the University of Nottingham.

Murray is an experienced independent investigator and has undertaken external reviews and inquiries in the public sector and with international non-governmental organisations.

His practice also includes work as an independent editor, writer and presenter, producing work to stimulate debate about complex professional and ethical issues.

Steve Clinch

Following a successful career in commercial shipping, Steve Clinch joined the UK’s Marine Accident Investigation Branch in 2004.

He was also the organisation’s chief inspector from 2010 until his retirement from the civil service in 2018.

He is currently a trustee director of Nautilus International and the Royal Alfred Seafarer’s Society.

Steve was also previously the General Secretary of the Marine Accident Investigators’ International Forum.

Dr Mike Durkin

Dr Mike Durkin

Dr Mike Durkin supports leaders in patient safety across England and globally.

He is senior advisor on patient safety policy and leadership for the National Institute for Health Research, Imperial College Patient Safety Translational Research Centre. He is also director of the Global Patient Safety Collaborative, launched in 2019 as a partnership between the World Health Organization and the UK Government.

Prior to this he held the position of national director of patient safety at NHS Improvement, from 2012 to 2017.

Mike is a visiting professor at the Institute of Global Health Innovation, Imperial College London, and the University of the West of England. He was awarded an OBE by the Queen in 2017 in recognition of his services to patient safety.

Farrah Pradhan

Farrah has a very strong commitment to ensuring the patient voice and user experience is integral to improving health outcomes and safer services for members of the public. This is evident in her work history in influencing the work of healthcare organisations to improve and maintain safe, effective, equitable and integrated care for members of the public.

She successfully uses her advocacy skills to influence patient and public involvement in developing health policy, research and clinical guidance. As her career has progressed and having been exposed to safety concerns in the delivery of healthcare, she has broadened her knowledge in patient safety, safety management and improvement initiatives. Having completed a Masters qualification in Patient Safety (Imperial College London, 2021) Farrah's areas of interest are the principles of safety-II (learning from what goes well in healthcare) and resilience engineering.

Dr Joe Rafferty

Dr Joe Rafferty is the Chief Executive of Mersey Care NHS Trust. Previously, he was Director of Commissioning Development for NHS North West, working on design and delivery of elements of the Liberating the NHS commissioning reforms. Included in the HSJ’s list of 100 most influential people in health, Joe has been championing Expert by Experience as a major driver for transformation.

Jennie Stanley

Jennie Stanley qualified as a registered nurse in 1988 and went on to specialise in emergency nursing. Jennie became a whistleblower following patient safety concerns in her organisation. She endured a four-year legal battle to vindicate her name, engaging during the process with the High Court. Jennie was Lead Nurse (UK) at Patients First from its inception in 2011 until 2017. She runs her own training company which is regularly utilised by NHS trusts. Jennie brings a whistleblower’s perspective to the panel and believes very strongly in making the patient the first concern.

Professor Patrick Vernon, OBE

Chair of Citizens' Partnership for HSIB

Patrick has recently been appointed Independent Non-Executive Director of Birmingham and Solihull ICS where he leads on inequalities, and Non-Executive Director for Hertfordshire NHS Trust and Associate Director for Connected Communities for the Centre for Ageing Better. He is also a Clore and Winston Churchill Fellow, Fellow of Goodenough College, Fellow at Imperial War Museum, Fellow of Royal Historical Society, and former associate fellow for the department of history of medicine at Warwick University. and Patrick was awarded an OBE in 2012 for his work on tackling health inequalities and ethnic minority communities.

In 2018 he received an honorary PhD from Wolverhampton University and was selected as one of the 1000 Progressive Londoner by the Evening Standard. In 2019 he was awarded a lifetime achievement award for campaigning and advocacy work by the SMK Foundation.

In 2020 Patrick was selected by British Vogue as of Britain’s top 20 campaigners and was included in the 2020 Power list of 100 influential Black People in Britain. In 2020 he was co-author of 100 Great Black Britons. In 2020 Patrick established the Majonzi Fund which is providing small grants to families and community organisations to organise commemoration events for individuals from BAME communities who have died of covid-19 over the last 19 months. Finally, Patrick is a broadcaster and writes blogs and articles for the national and international media on healthcare, cultural heritage, and race. In August 2021 Patrick was appointed by Wolverhampton University as Honorary Professor of cultural heritage and Community Leadership for the Department of Community Development.

Richard von Abendorff

Richard von Abendorff is a family campaigner and advocate for learning and robust action in healthcare after avoidable harm. Previously he spent 17 years working in services for frail older people and dementia care in health, social services and home settings. He campaigns in a voluntary capacity for learning and action in end of life care, following many serious avoidable failings in his mother’s palliative care in hospital. Richard wants to ensure incident reporting leads to robust evidenced action and also an end of what, too often, becomes an attack on the messengers of harmful incidents.
